Sample enrolment form
In preparation for the release of the VET Information Standard, an updated sample enrolment form is now available.
The form has been revised to capture the new and updated data elements introduced under the new standard and can be downloaded and tailored to suit individual RTO needs.
What’s changing
The VET Information Standard enrolment form has some key differences when compared with the AVETMISS 8.0 enrolment form version including:

Updated gender & disability classification schemes to align with Australian Bureau of Statistics (ABS) classifications

Slight modification to the language spoken at home question

Inclusion of a student’s temporary address while they are studying

Introduction of an additional study reason to capture a student’s intent to attain a VET in Schools outcome

A revised privacy statement to support the new reporting requirements
While the changes to the form are relatively minor, RTOs are encouraged to:
Familiarise themselves with the updated fields and requirements ahead of transition
Review whether these new data fields are currently being captured
Consider updating enrolment forms including the Privacy Notice
This will assist RTOs in preparing for transition and ensure the information collected and ultimately reported is correct.
Please note: Existing AVETMISS-compliant enrolment forms will remain valid for RTOs continuing to report under AVETMISS during their transition period.

How to use this form
The sample enrolment form contains the minimum set of questions that RTOs should ask each student when enrolling to help meet their data reporting requirements. These questions can be incorporated into physical or online enrolment forms.
It is highly recommended that RTOs maintain the question sequence and wording if developing their own enrolment forms. It is not compulsory for a student to answer all questions.
Former students who return to enrol in additional training must complete a new enrolment form to ensure their information is current and accurate. They must also be provided with a privacy notice so they are aware of the purposes for which their information may be collected, used or disclosed. It is also recommended that RTOs have a process to ensure continuing students' information is kept up-to-date.
To meet the Data Provision Requirements 2020, RTOs must provide all the data available to them about enrolled students, and the training activity they deliver to enrolled students. The sample enrolment form is designed to assist registered training organisations (RTOs) collect and submit data compliant with the national VET Information Standard.
In addition to the minimum VET Information Standard questions, the sample enrolment form also includes:

The name of the VET Information Standard element (in italics below each question) which corresponds to the question.
This has been provided to assist RTOs and student management system suppliers but does not have to be used on actual enrolment forms.

The minimum mandatory content for inclusion in a privacy notice so that students are aware of the purposes of which their personal information may be collected, used and disclosed (refer Schedule 1 of the Data Provision Requirements).
It is the responsibility of RTOs to ensure students who provide an RTO with personal information are reasonably aware the information may be collected, handled or disclosed.
